Individual Living Assessments for comprehensive care and living needs.
Consumer and Family Orientation: Inform about rights, services, and funding.
Individual Program Planning and Development: Identify goals and develop support plans.
Service Coordination: Advocate, provide resources, and education.
Crisis Intervention: Respond and follow-up on acute needs.
Referral: Identify and assist in accessing community resources.
Consultation: Collaborate with other providers for quality care.
Reports and Record-keeping: Maintain timely documentation.
Confidentiality: Adhere to strict confidentiality regulations.
Resource Development: Secure necessary care and services, ensure housing safety.
QUALIFICATIONS Education and Training Requirements
High school diploma plus demonstrated experience.
1-2 years of related experience working with individuals who are medically disabled highly preferred.
Bachelor's degree preferred.
Competence in working with individuals who are medically disabled,
Must have valid driver’s license, motor vehicle and automobile liability insurance coverage
SALARY & BENEFITS
Competitive Salary $20.00 per hour plus, depending on education and experience
