Horizon Adult Health Care is Hiring a Nurse Director (RN) Adult Day Director Near Whitley, KY

Reports to:

General Summary:

Under the direction of the Horizon PACE Director, the Adult Day Health Care Center director

will manage the operation of the adult day health care center by updating client clinical records, managing staff, monitoring holistic care, managing errand lists, ensuring a safe environment, providing documentation of center’s activities and ordering nursing and office supplies to enable staff to provide quality client care and maintain the client’s optimal level of health.

QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill and/or ability required.

1. Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.

2. Must have a valid driver’s license, proof of insurance and have a reliable means of transportation.

3. Education: Degree in nursing with an unrestricted RN or LPN licensure in the State of Kentucky, or an unrestricted license from a surrounding state with compact/multistate ability.

4. Experience: A minimum of three year’s successful experience in working with the frail or elderly population is preferred.

5. Competency: Position specific competencies for the Adult Day Health Care Director will be met prior to assuming participant care.

6. Skills and Knowledge:

o Thorough knowledge of current concepts and practices of health and social services for the elderly, particularly in home and community-based services, and the interdisciplinary team model.

o Thorough knowledge of or ability to learn managed care environment; including claims management, management of statistical and medical indicators, and managed care utilization trends and implications for the PACE program.

o Working knowledge of health care delivery and financing systems, including Medicaid, Medicare, waiver programs, prospective payment systems, and the Health Management Organizations

ESSENTIAL FUNCTIONS:

1. Responsible for the oversight and administration of the adult day health care to include effective development, planning, organization, administration, fiduciary oversight, and evaluation.

2. Assure employment of qualified staff and development of personnel practices which promote high standards of participants' well-being.

3. Directs, supervises and evaluates the performance staff as needed.

4. Makes all reasonable efforts to ensure the provision of staff orientation and staff education, and that job responsibilities, authorities and accountability of all direct reports are defined and understood.

5. In conjunction with the Executive Director, Medical Director, Horizon PACE Director, and Quality Improvement Coordinator, responsible for maintaining compliance with all State and Federal program requirements, obtaining and maintain certification under Centers for Medicare and Medicaid Services (CMS) as well as any other State or Federal law or rules.

6. In conjunction with the Executive Director, Horizon PACE Director, and Medical Director, responsible for maintaining compliance with all state and federal program requirements. Monitors medical and specialty services utilization to strive to ensure provision of quality care to participants.

7. Responsible to assist with the development and implementation of Horizon PACE's Strategic Plan.

8. Assists in program development and preparation of Horizon PACE's short-range and long-term plans, budgets and related information systems based on broad organization goals and growth objectives.

9. Recommends and implements Horizon PACE's operational policies, procedures and protocols for direct participant care and support services.

10. Has oversight and ensures delivery of comprehensive services to the populations served by Horizon PACE. Monitors the growth of care delivery and services as the participant census grows and makes adjustments when appropriate.

11. Meets program goals for target enrollment; is accountable for quality of services provided and program financial performance.

12. Participates in the development, and monitoring Horizon PACE's compliance with staffing guidelines including use of temporary/contracted staff.

13. Responsible for the management and development of appropriate quality improvement systems and programs for Horizon PACE.

14. Responsible implementation of appropriate Quality Improvement programs. Participates regularly in quality assurance/quality improvement committees as part of the overall quality improvement program, in conjunction with the Quality Improvement Coordinator strives to ensure that appropriate indicators are established and necessary outcomes are attained. Participates in the quality assurance program for participant care and staff performance

15. Establishes positive working relationships with health and social service providers.

16. In conjunction with the Executive Director, Medical Director, Horizon PACE Director, and Quality Improvement Coordinator, maintains the stability and reputation of Horizon PACE by helping to ensure that all activities and operations are performed in compliance with local, state, and federal contracts, regulations, laws, accreditation, protocol, licensing and certification requirements governing PACE organization operations.

17. Assists with oversight of Horizon PACE’s operational compliance with applicable Occupational Safety and Health Administration (OSHA) regulations and agency policies and procedures.

18. Makes reasonable efforts to ensure that a safe and clean environment is maintained for staff and program participants.

19. Protects privacy and maintains confidentiality of all company procedures, results and information about employees, participants and families.

20. Continues professional development and participation in local, state and national programs related to PACE. Participates in continuing education classes and any required staff and training meetings. Maintains professional affiliations and any required certifications.

21. Ensures that all personnel are assigned duties based upon their education, training, competencies and job descriptions. Responsible for the recruitment, selection and training of adult day health care staff.

22. Establish and convey performance expectations, assess performance, provide constructive feedback and conduct timely performance evaluations.

23. On a rotational basis, may provide evening and weekend on-call administrative coverage.

24. Strives to ensure the development, implementation, and enforcement of all policies and procedures, including participant rights.

25. Establishing and maintaining liaison relationships and communication with facility staff and services, with support services and community resources and participants. Responsible for the growth and viability of the program through development, outreach, enrollment and expansion to new service market areas needed and approved.

26. Acts withing the scope of his or her practice to perform skilled nursing services as needed for adult day health care attendees.

27. The ADHC director, or their designee, will inventory medical supply and medication expiration dates, discard and replace expired items in the Adult Day Health Care.

28. Performs related duties as assigned

GENERAL RESPONSIBILITIES

2. Is knowledgeable of participant rights and ensures an atmosphere which allows for the privacy, dignity and well-being of all participants in a safe, secure environment.

3. Supports, cooperates with and implements specific procedures and programs for:

a. Safety, including standard precautions and safe work practices, established fire/safety/disaster plans, risk management and security, report and/or correct unsafe working conditions, equipment repair and maintenance needs.

b. Confidentiality of all data, including participant, employee and operations data and reports breaches immediately to the HIPAA Privacy or Security Officer.

c. Quality Imprement and compliance with all regulatory requirements.

d. Compliance with current law and policy to provide a work environment free from harassment and all illegal and discriminatory behavior.

4. Supports and participates in common teamwork:

a. Cooperates and works together with all co-workers; plans and completes job duties with minimal supervisory direction, including appropriate judgment.

b. Uses tactful, appropriate communications in various situations.

c. Follows up as appropriate with supervisor, co-workers or participants regarding reported complaints, problems or concerns.

d. Promotes positive public relations with participants, caregivers, and guests.

e. Adheres to attendance policies, including clocking in and out for meal time, following break schedule, and seeking supervisor’s approval prior to working overtime.

f. Completes requirements for in-service training, uniform and dress codes including personal hygiene, and other work duties as assigned.

5. Assist supervisor with various tasks to support department operations as directed; may also be assigned special projects.

6. Perform tasks which are supportive in nature to essential functions of the job, but which may be altered or re-designed depending on individual circumstances.

7. Assists participants and caregivers in filing grievances.

Work Environment

1. Visual, Hearing and Communication Requirements - Requires corrected vision and hearing to normal range. Must be able to communicate verbally with all staff, caregivers, participants, and community at large.

2. Pressure Factor - Requires working under stressful conditions. Working conditions may be noisy and crowded with fluctuating indoor temperatures. Moderate pressure to meet scheduled appointments. Subject to people that may have the potential for verbal or physical aggression.

Physical Demands

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.

1. Physical Requirements – Shall be in good mental and physical health, and capable of performing assigned tasks. Employee positions require strenuous activity including pushing, pulling, lifting of 40 pounds or more, standing for long periods, extensive walking, and movement at different position levels. Employment is contingent on the ability to perform these duties with no limits or restrictions according to regular work schedule.
