We provide a wide array of trauma-informed healthcare, emergency services, and support to nearly 20,000 individuals each year, and we are committed to making our programs more accessible to all.JOB TITLE: Case ManagerHOURS: Full time (35 hours/week)PROGRAM: Maternal & Child Health Services, Young Parents' ProgramOVERVIEW: Unde r the direct supervision of the Program Director, the Case Manager will use strength-based, individualized, trauma-informed approaches to meet young parent's needs. A Case Manager will:SPECIFIC DUTIES: Provide case management, information and referrals Assist young parents and their children in accessing community resources Provide parent education services within homes, program site schools or other programs Provide parent education and life skills development Provide transportation to young parents and children Promote the achievement of objectives by and for young parents and children that improve their health Record, track and maintain participant data as required Participate in group support services Outreach to priority populations through community events, community providers Participate in on-the-job training as required by funders and Health Imperatives Attend meetings Other duties as assigned by the Director/Supervisor.RequirementsREQUIRED QUALIFICATIONS :Bachelor's degree in social work, early childhood education, human services or related field or Associate's degree and one to three years working with pregnant or parenting young parents. Must demonstrate ability to assist vulnerable populations with various barriers access healthcare, housing, legal, educational and financial systems. Travel required. Reliable transportation, valid MA driver's license, auto insurance, registration and a safe driving record required. Ability to maintain flexible schedule in accordance with program needs; i.e. some evenings or Saturdays may be required.
