What You'll Do as a Care Manager

  • Meet with individuals in their homes to complete assessments and provide coordination of services to improve or maintain the member’s independence and quality of life.

  • Complete strength-based assessments with seniors and adults with disabilities in their homes upon enrollment with monthly contacts and quarterly visits.

  • Develop customized care plans that focus on individuals’ preferences and support their health and safety needs.

  • Maintain relationships with members, their support, and with acute and primary care providers to ensure the best possible health outcomes for members.

  • Coordinate functional and social support to promote member independence and mitigate risk.

  • Work in partnership with a Nurse Care Manger to collaborate on a member’s plan of care and work together to monitor the member’s overall well-being.

  • Use the Long Term Care Functional Screens (LTCFS) to gather information about the member and complete LTCFS in a collaborative manner with the member, family, and informal/formal supports.

  • Attend meetings and trainings as required by the LTCFS Division and participate in LTCFS quality assurance activities.

  • Maintain LTCFS certification by completing and passing DHS LTCFS continuing skills testing.

  • Other duties as assigned.

Minimum Requirements:

  • A completed Bachelor’s degree.

  • One or more years of paid experience working with our member population of adults who are intellectually/developmentally disabled, physically disabled, and/or the frail elderly.

  • A valid Driver’s License, access to a reliable vehicle, and automobile insurance coverage at time of application and throughout employment.

  • Willingness to attend required in-office meetings, events, and trainings as requested by Care Management leadership.
