We are looking for an indivual who can take on the roll as a dir of social work at our SNF. This job includes but is not limited to,

-setting up care plan meetings

-working with other departments to coordinate best plan of action for each resident

-strong follow up, and people skills

-evaluting residents using the MDS assessment

-working well with assited livings as well as home care agencys to insure a safe and timely discharge for each indivual resident.
