POSITION SUMMARY:
The Health Outreach Coordinator holds a pivotal role in the department, coordinating and executing all aspects of outreach for the health education services provided. This multifaceted position involves the establishment, organization, and successful execution of community outreach and health events tied to grant activities. In addition to outreach, this role assumes responsibility for the preparation and facilitation of health screening events at the Caridad Center and various community partner sites. Furthermore, the Health Outreach Coordinator plays a critical role in follow-up activities, ensuring that individuals with abnormal findings receive follow-up calls and support. This includes reaching out to patients and community members to engage them in educational activities and confirm their attendance. This position is essential for bridging the gap between health education services and the community, making a significant impact on public health and well-being.
ESSENTIAL FUNCTIONS & RESPONSIBILITIES:
· Identifies and organizes health awareness events and educational activities to support departmental deliverables, objectives, and goals.
· Plans, coordinates, and implements event logistics including but not limited to venue selection, catering, speakers, displays, screenings, facilities, and equipment arrangements.
· Manages inventory for health screening materials to ensure their availability and organization including material distribution and post-event material retrieval.
· Engages in effective communication with management and staff, seeking input and collaboration in event planning and evaluation.
· Assists in the development of budgets for health-related events and adheres to budgetary constraints when procuring services for these events.
· Maintains accurate records and safeguards the confidentiality of data collected during health assessments, on-site screenings, and health promotion activities.
· Actively participates in and attends all community events, ensuring comprehensive coordination from event setup to breakdown.
· Identifies potential community partners for health education initiatives and executes Memorandums of Understanding (MOUs) when necessary.
· Ensures existing MOUs are renewed annually.
· Monitors the department's health education agenda to ensure the proper coordination of external educational activities required to meet monthly deliverables.
· Implements outreach strategies to recruit and schedule patients and external participants for educational activities and events.
· Submits monthly reports on time.
· Attends meetings and training sessions as required.
· Demonstrates a deep commitment to health equity, including addressing racial equity and social determinants of health.
· Fulfills other duties as assigned.

QUALIFICATIONS:
· Bachelor's degree in a related field, such as public health, community health, or a relevant discipline.
· Proven experience in community outreach, health education, or a related field, preferably in a healthcare or nonprofit setting.
· Strong organizational and project management skills to effectively coordinate and execute outreach initiatives and health events.
· Trilingual (English/Creole/Spanish) Required.
· Proficient using various systems including Microsoft Office (Word, Excel, PowerPoint) and Canva (or other design software) required.
· Must demonstrate strong interpersonal, oral, and written communication skills.
· Effective collaboration and communication skills.
· Ability to work independently.
· Ability to handle complex situations professionally.
· Willingness to take on additional duties and responsibilities as assigned, adapt to changing priorities, and work on special projects as needed.
WORKING CONDITIONS:
The Health Outreach Coordinator works at Caridad Center, outpatient centers, and external organizations. This position requires frequent travel to various external sites within Palm Beach County, thus requires reliable transportation suitable for transporting supplies such as tents, tables, chairs, etc. While performing the duties of this job, this candidate is regularly required to stand, sit, and operate basic technology accordingly. Lifting is required, including but not limited to setting up tents, tables, and chairs for events. This position may require working irregular hours.
