We are currently hiring a full-time 245D Designated Manager to join our team. This is a hybrid position. Most hours can be completed remotely working from home with in-office time as needed for various projects, meetings, and licensing audits. Face-to-face meetings with participants/teams is very rare but may be necessary to complete service support documentation or to provide training as needed. No on-call, and no direct care.
The Designated Manager role is responsible for oversight of person-centered service planning and delivery, staff training and supervision, and licensing compliance related to 245D basic and intensive Individualized Home Support services.
Responsibilities:
· Stay abreast of 245D licensing statutes as they apply to basic and intensive services
· Consistently evaluate programmatic compliance and take actions necessary to ensure licensing requirements are met
· Prepare for, respond to, and lead state licensing audits
· Develop and/or modify forms, policies, documents, workflows as needed
· Complete inquiries and intakes
· Use person-centered principles when working with participants and their teams to plan and develop coordinated support plan addendums, individual abuse prevention plans, intensive support self-management assessment, individual goals and outcomes, and other service-related documents.
· Set up manage participants’ budget/spending plans for each service plan year and provide communication regarding spending patterns as needed.
· Conduct semi-annual reviews with participants to ensure positive support strategies are appropriate
· Assist participants and their teams with revisions/modifications to their service support/spending plans throughout the year
· Assist with recruiting and onboarding new staff as needed
· Manage third-party training system, develop/update curriculum as needed, and manage all new hire and annual staff training
· Develop and monitor data-tracking for participants who use intensive services and compile/summarize data for progress reviews/reports as needed
· Track, document, investigate any reported incidents
· Assist with other departmental projects as assigned
Qualified candidates must meet one of the two qualifications:
1. Bachelor’s or Associate’s degree in a human services field, and a minimum of 2 years of experience working with persons with disabilities or persons age 65 and older.
OR
2. 4 years of full-time experience working with persons with disabilities or persons age 65 and older
Plus have these additional qualifications
· 3 years of previous supervisory experience in the human services field
· Extensive working knowledge of 245D services, statutes, and compliance requirements
· Ability to pass a DHS background check
· Valid driver’s license, clean driving record, auto insurance, and reliable transportation
