Social Worker
StartDate: 3/3/2025 Pay Rate: $1800.00 - $2600.00
POSITION SUMMARY – Social Worker
POSITION DUTIES –
- Provides coverage as needed at any of our facilities.
- Provides comprehensive psycho-social assessment, intervention, management, support, and assess for discharge planning and aftercare needs of the at-risk patient population and their families.
- Addresses social determinants of health and works collaboratively with the multidisciplinary team.
MINIMUM REQUIRED QUALIFICATIONS – LCSW or LMSW (NY); BLS; Post-master's experience, preferably in a hospital or healthcare environment.
PREFERRED QUALIFICATIONS – 3 years in a hospital and/or healthcare setting
LENGTH OF ASSIGNMENT – 13 weeks
SHIFT / HOURS PER WEEK – 5 8hr days
START DATE – ASAP
