The Program Coordinator will be member of Ascentria Developmental Disabilities Services Leadership Team. This program supports individuals and families (ages 6 ) in their home.
Responsibilities:
- Demonstrate a Trauma Informed, human centered and strength-based philosophy, by treating all clients, families and collaterals in a non-judgmental and respectful manner while building upon their strengths and experiences
- Consistently demonstrate discretion and independent judgment with regard to assessing and making decisions around the best interest of individuals and families receiving supports
- Manage individual client budgets to ensure disbursement of funds meets all regulatory standards
- Participate in the IEP and ISP process/ responsible for treatment planning and implementation of interventions
- Assure that required documentation is complete and professional, that it is done in a timely manner and meets contractual guidelines
- Coordinate weekly communication with families and staff in a professional manner
- Participate in on call responsibilities in a calm and professional manner while insuring the safety of the individual involved
- Inform Program Manager of any incidents or concerns while utilizing professional judgement
- Lead team while insuring the highest quality of services for both individuals and families
- Provide creative problem solving alternatives in difficult and crisis oriented situations
- Maintain open and clear communication with all staff and clients\families
- Function consistently with all agency policies including confidentiality and client rights and responsibilities
- Assist with PQI efforts and implementation of outcome measures which assess programmatic impact on client’s lives
Qualifications:
- Bachelor’s Degree in related preferred but will considerable comparable experience working in a community and/or residential based setting
- Ability to obtain licensure within 6 months of employment required. ASWB licensure according to education and experience level {LICSW LCSW, LSW or LSWA}
- Minimum of 1 year management/supervisory experience preferred.
- Knowledge of DDS ISP, and IEP Process, PBIS, and Charting The Life Course preferred
- Ability to provide strong leadership and supervision to staff while being supportive and positive.
- Ability to effectively delegate tasks, train staff, and recognize achievements of both staff and individuals served.
- Ability to set priorities and manage multiple tasks while meeting deadlines.
- Ability to manage conflict, stress, and respond to changing environments.
- Experience with managing crisis situations.
- Ability to implement treatment plans and set limits and manage client behaviors.
- Ability to provide on-call coverage and work a flexible schedule, including nights and weekends as needed.

- Valid driver’s license and proof of automobile insurance is required to transport clients throughout the community in personal vehicle and/or Ascentria vehicles.
- Strong computer, Microsoft Office skills required.
