The Director of Case Management is responsible for the oversight of Case Management unit, including but not limited to clinical resource management, discharge planning activities, patient advocacy, clinical social work and best practice in medical necessity determination and concurrent review. Accountable for the execution, adaptation and outcome of care management as prescribed in the initial Utilization Review Plan with regard to resources, appropriateness of care and adherence to a designated Geometric Length of Stay (GLOS) for each patient.
Essential Functions
Develops and implements departmental goals, plans, and standards consistent with the clinical, administrative, legal, and ethical requirements/objectives of the organization. Directs and evaluates departmental operations, including patient care delivery, information technologies, service level determination, and complaint management, to achieve performance and quality control objectives.
Plans and monitors staffing activities, including hiring, orienting, evaluating, disciplinary actions, and continuing education initiatives. Prepares, monitors, and evaluates departmental budgets, and ensures that the department operates in compliance with allocated funding. Coordinates and directs internal/external audits. Creates and fosters an environment that encourages professional growth.
Integrates evidence-based practices into operations and clinical protocols.
The Director is responsible for the overall direction and management of these areas, including planning, organizing and directing all activities, staffing, performance improvement in the delivery of clinical services (such as LOS reduction), and reporting needs, as well as government and regulatory reporting.
Provides direct patient care including assessment, diagnosis, treatment and technical procedures to a group of normal-to-high risk infants. Manages neonatal caseload with physician consultation and collaboration. Serves as patient and family advocate in identifying healthcare needs and actions for neonatal cases. Coordinates pre- and post-operative care activities for normal-to-high risk infants. Supports neonatal units during crisis situations. Utilizes planning, implementation, and evaluation in formulating neonatal diagnosis and intervention.
Qualifications:
- Minimum three (3) years of experience in case/utilization leadership in an acute care setting
- BSN, Masters preferred
Job Type: Full-time
Pay: $118,000.00 - $150,000.00 per year
