We are seeking a skilled Home Care Nurse to join our team. In this role, you will provide essential nursing services to patients in their homes, ensuring they receive the highest level of care tailored to their individual needs. Responsibilities
- Develop and implement individualized care plans for patients.
- Monitor and assess patient health status regularly, documenting changes and communicating with healthcare providers.
- Assist patients with daily living activities and personal hygiene as needed.
- Provide specialized care for patients with dementia and other chronic conditions.
- Administer medications and treatments as prescribed by physicians.
- Educate patients and families on health management and wellness strategies.
- Maintain accurate records of patient progress and treatment plans.
Requirements
- Valid nursing license in the state of practice.
- Experience in home health care or a related field preferred.
- Strong knowledge of patient monitoring techniques and care plan development.
- Compassionate approach to senior care and dementia management.
- Excellent communication skills for effective interaction with patients and families.
