Neurosurgery/ Ambulatory Nursing/ Boston

Job Summary

The Transitional Care Team (TCT) is an integral member of the TCT and co-leads in programmatic development and implementation. Founded on a human caring framework and utilizing relationship-based care principles, the model promises to transform practice and the care delivery for neurosurgical patients.

With recognition, the patient is at the center of the team, the nurse works collaboratively across the inpatient and outpatient clinical teams, managing, coordinating, and delivering transitional care services.

Supplemental patient education will be provided in the form of a discharge bundle, streamlining the discharge process and further ensuring patients receive appropriate education, care and services to facilitate a seamless transition home.

The Transitional Care RN will also play a vital role in access to care and help support the department and division in ambulatory clinical patient triage.

May include weekend rotation.

Qualifications

Education

Bachelor's Degree Nursing required. Can this role accept experience in lieu of a degree? Yes

Licenses and Credentials

Registered Nurse, Commonwealth of Massachusetts, required.

Experience

  • 1. Minimum of 2 years of recent (within 12 months) clinical nursing experience in an Ambulatory Clinic/Practice setting required. Experience only in an ambulatory procedural and/or emergency room not sufficient.
  • 2. Minimum of 2-3 years of Neurosurgical experience required.
  • 3. Previous Neurosurgical ambulatory experience with a demonstrated ability to educate patients and families around current treatments and medications required.
  • 4. Experience in communications and customer service strongly preferred.
  • 5. Experience with team-based care preferred.
  • 6. Case Management experience preferred.
