What are the responsibilities and job description for the IR Clinic Coordinator position at Cape Radiology Group?

JOB EXPECTATIONS

Duties: The IR Clinic Coordinator will assist Group’s physician employees in the professional management of their clinical patients by supporting patient-facing administrative functions pertaining to the CRG IR Clinics as well as CRG patient care at hospital sites of service.

· Answer dedicated IR Phone Number at all times from 7:30-4:30. Goal is for callers to reach a live person every time they call. Resolve administrative issues. For clinical issues, take complete information from caller and relay to appropriate APP or Physician with correct contact information to facilitate follow-up.

· Review all faxes arriving to dedicated IR Fax Number and appropriately triage.

· Schedule all new patient consultations in CRG IR Clinics according to scheduling guidelines (appt duration, provider preference, expeditious appts, etc). Verify all patient information at time of scheduling (contact info, insurance, etc). Work directly with Chief of IR when scheduling questions arise.

· For new patient physician-referrals, ensure that referring office sends all information necessary and available for the referral and that it is received from the referring office prior to the clinic appointment. This includes but is not limited to clinical notes, insurance information, patient contact information, imaging, labs, etc. Follow-up with referring office routinely until this information has been received prior to clinic visit.

· For new patient physician-referrals, reference disease-specific clinical workflow guidelines. If necessary items are not available, work with clinic providers (APP/physician) to order necessary tests, labs, imaging, etc. Ensure that all necessary information is available for review prior to clinic appointment.

· For new patient self-referrals, ensure that all designated self-screening questionnaires have been taken and that patient meets criteria for self-referrals (see disease specific guidelines) prior to scheduling clinic appointment.

· For all clinic patients, ensure that all appropriate patient questionnaires (see disease specific guidelines) are given to patient upon checking in for appointment to be filled out prior to being brought back from waiting room.

· Schedule follow-up care for all CRG IR patients.

o Schedule follow-up clinic appointments at time of current clinic appointment.

o Schedule follow-up imaging, needed both immediately and prior to future clinic appointments.

o Schedule follow-up labs, needed both immediately and prior to future clinic appointments.

o If future clinic appointments are changed, ensure that scheduled pre-clinic imaging and/or labs are changed as well.

o Support diagnostic pre-authorizations performed by those primarily responsible when requested (incl sending clinical documentation)

· Reschedule appointments and associated tests when necessary

· Verify patient care plan in finalized clinic notes

· Schedule therapeutic procedures at the appropriate hospital through the appropriate scheduling process for each site

o Communicate needed supplies/reps/equipment/room/analgesic requirements to appropriate point person at each site.

o Facilitate appropriate procedural pre-authorizations are obtained by sending all relevant clinical documentation to pre-authorization team

· Provide pre-operative patients with an appropriate information packet including pre-operative & post-operative instructions per procedure-specific guidelines.

· Coordinate all peer-to-peer reviews with insurance companies. Communicate time to provider. Gather all relevant clinical data and provide to provider in an easily digestible format prior to peer-to-peer call. Follow-up with provider regarding outcome of peer-to-peer and necessary next steps. Ensure additional steps are taken including submission of additional clinical documentation, scheduling of additional tests/clinic visits, etc.

· Send clinic notes to office of patient’s initial referring provider and PCP after every clinic visit. Ensure these are sent according to CRG protocol re timeliness and presentation.

· Coordinate with marketing team / physician liaison to ensure providers are available for necessary off-site meetings.

· Work with physician liaison to ensure timely communication with new consults and referring offices

· Actively participate in improving patient experience and workflows within CRG IR Clinics as well as improving communication between CRG IR Clinics & hospital procedural sites.

· Follow-up with no-show patients until they are recaptured or resolved.

· Maintain an up-to-date clinic visit patient database daily

· Formulate and submit to IR Chief a monthly report to include volumes, referring providers, and issues around clinic workflows
