Job Overview
The Designated Coordinator is a key leadership role responsible for supervising and supporting direct care staff while ensuring the delivery of high-quality, person-centered services. The Designated Coordinator plays an integral part in compliance with federal and state regulations, 245D requirements, and organizational policies, fostering a safe and supportive environment for individuals receiving care.
Key Responsibilities
- Compliance Oversight: Ensure compliance with 245D licensing requirements and all relevant federal, state, and organizational regulations.
- Staff Supervision: Train, schedule, and evaluate direct care staff, providing ongoing support to enhance service delivery and adherence to care plans.
- Service Management: Oversee and evaluate the effectiveness of service delivery, methodologies, and progress toward individuals' goals and outcomes.
- Communication and Coordination: Serve as the primary point of contact for individuals, guardians, case managers, behavior analysts, and other team members, ensuring a collaborative approach to meeting client needs.
- Data and Reporting: Maintain accurate and detailed records, summarizing data for compliance and service evaluation reports.
- Client Support: Coordinate appointments, respond to emergencies, and provide direct care when needed.
Qualifications
To qualify for the Designated Coordinator position, candidates must meet the following requirements:
- Education and Experience (One of the following):
- Bachelor’s Degree in human services, education, or health field and one year of full-time direct care experience with individuals with disabilities or individuals aged 65 .
- Associate Degree in a related field and two years of full-time direct care experience.
- Diploma in a related field from an accredited institution and three years of full-time direct care experience.
- Minimum of 50 hours of training related to human services and disabilities, plus four years of full-time direct care experience under qualified supervision.
- Additional Requirements:
- Current Driver’s License and ability to travel to various locations in the metro area.
- Experience with individuals with high behavioral support needs (preferred).
- Proficiency in 245D compliance, Microsoft Office, and Teams.
- Strong organizational, written, and oral communication skills.
