POSITION SUMMARY: Provide a variety of support services to individuals and families who are homeless, near-homeless, and/or in crisis; provide linkage to resources and services; provide financial assistance for eligible persons; conduct public relations and fundraising activities; and complete program documentation accurately and timely. Provide support services through the HOPE, Veteran’s RRH program, THRIVES, HART, and Stability Assistance. Assist with the screening of walk-ins, phone calls, etc to ensure referrals to the appropriate programs are made.
ROLE AND RESPONSIBILITIES
- Responsible for completing the agency intake form and inputting it into Clarity
- Provide ongoing support to clients and document via home visits, phone calls, and written correspondence with goal attainment of 65% or above
- Maintain a non-judgmental attitude toward clients and other CNCAP staff
- Ensure clients are assessed through the internal process to determine possible assistance through resources
- Create family personal plans to develop short and long-term attainable goals with families
- Ensure files are complete with the necessary paperwork for veteran families participating in the program
- Adhere to all CNCAP-related policies and procedures
- Promote the program and agency in a positive and professional manner
QUALIFICATIONS
- High school diploma or GED required (Bachelor's degree in human services or related field preferred)
- Valid driver's license
- 1-3 years of experience in human services or a related field

- Knowledge of word, excel, PowerPoint, and databases
- Excellent time-management and organizational skills
- Demonstrated ability to work with others
SCHEDULE: Monday-Thursday: 7 am - 5 pm or Monday-Friday: 8 am - 4 pm
REPORTS TO: Family Outreach Director
JOB STATUS: Full-Time, 40 hours per week
PAY: $16.50 - $18.25 (depending on qualifications)
