Position Overview:
We are seeking a Licensed Mental Health Counselor (LMHC) or Licensed Master Social Worker (LMSW) to join our team for an Intensive Crisis Stabilization Center (ICSC) opening in June. The ideal candidate will be passionate about integrated healthcare and committed to providing exceptional care to our patients. As a practitioner at our clinic, you will collaborate with our multidisciplinary team to assess, diagnose, and treat individuals with mental health concerns.
Responsibilities:
- Conduct comprehensive assessments of recipients in crisis, determining mental health needs and appropriate course of action
- Providing short-term therapeutic interventions, including crisis counseling, de-escalation and emotional support
- Coordinate with other crisis team members, medical processionals, and external agencies to ensure appropriate care
- Provide resources and referrals to recipients for ongoing therapy, mental health treatment and support services
- Maintain accurate, timely documentation of recipient interactions, assessments and individual service plans in accordance with agency and legal requirements
- Offer support and consultation to family members and caregivers, providing education about mental health resources and coping strategies.
- Participate in regular team meetings to discuss cases, share resources and ensure the delivery of high-quality care
- Ensure care meets all Trauma Informed Best Practice criteria
- Completing psychosocial evaluations
- Consistent follow-up with all recipients, and as appropriate families, to promote engagement and retention in services and to ensure all barriers to service access and use are addressed.
Qualifications:
- Master's degree in Counseling or related field
- Current LMHC/LMSW license in NYS
- Experience working in an integrated healthcare setting preferred
- Strong communication and interpersonal skills
- Ability to work effectively in a team environment
