What are the responsibilities and job description for the RN for Intensive Crisis Stabilization Center position at Champlain Valley Family Center?
Champlain Valley Family Center is a growing Integrated Behavioral Healthcare Clinic. We are looking to hire RN's for our Intensive Crisis Stabilization Center that is opening in June, for all shifts and Full time or part time availability.
Responsibilities include:
- Complete intake process with new recipients using EHR
- Conduct triage and assessments with recipients
- Complete safety plans with recipients
- Formulate individual service plans with recipients
- Completes discharge plans and summaries with recipients
- Conduct wellness rounds to ensure recipient safety
- Assess and interprets vital signs, physical indicators, lab values, recognizes abnormal levels and intervenes appropriately.
- Assesses recipients nursing care needs, plans, implements and evaluates nursing care for recipients, documents and reports to proper authorities, and makes appropriate referrals to provide continuity of care.
- Maintains essential nursing records and reports
- Responsible for all controlled substances, toxic or dangerous drugs and medical supplies and responsible for ensuring medications and supplies are ordered and on hand.
- Administers medications to recipients, including intramuscular and subcutaneous injections and oral and topical medications as prescribed.
- Responsible for Medication Administration Record
- Adheres to nursing policies and nursing practice standards including responsibility for professional development
- Assess level of withdrawal from substance use using CIWA scale and/or COWS scale
Requirements:
- Valid Registered Professional Nurse (RN) license in NY State
- Excellent communication and interpersonal skills
