Responsibilities and Expectations:

• Develop and implement care plans that meet the unique needs of each resident.
• Collaborate with interdisciplinary teams to ensure effective delivery of services.
• Stay informed about changes in MDS regulations and best practices.
• Train and support staff members on MDS processes and procedures.
• Participate in quality assurance initiatives to drive continuous improvement.
• Maintain clear and respectful communication with residents and families.
• Work with the billing team to ensure accurate alignment with reimbursement requirements.
• Promote a positive and supportive work environment.

Qualifications:

• Current RN license in the state of Colorado
• Bachelor's Degree in Nursing (BSN) preferred
• MDS Certification preferred
• Minimum 2-3 years of experience as an RN in long-term care or skilled nursing community
• Proven experience with MDS assessments and care planning
• In-depth knowledge of Medicare and Medicaid regulations
• Strong clinical assessment skills and attention to detail
• Proficiency in electronic health record systems and MDS software
• Excellent communication and interpersonal skills
• Demonstrated ability to manage time effectively and prioritize tasks
• Thorough understanding of the Resident Assessment Instrument (RAI) process
• Ability to work collaboratively in a fast-paced healthcare environment
