Who You Are
As a Case Manager for Housing Stability at CAC, you will play a vital role in supporting heads of households and adults in maintaining stable housing. Working closely with the Client Services Team, you'll assist with family enrollment, address housing challenges, and provide coaching on essential topics such as financial wellness, time management, self-care, and goal setting. The ideal candidate will be an innovative, outgoing self-starter with a passion for helping families and children while building the Family Stability program in partnership with internal stakeholders. Strong emotional intelligence and a commitment to growing CAC’s programs is essential in fostering the personal and professional growth of parents and households, ensuring their long-term housing stability.
What You'll Do
- Enrollment: Assist with the enrollment and recruitment of families for The United Way Family Stability Program criteria.
- Collaboration: Acts as a liaison amongst internal teams at CAC to ensure that all clients are receiving comprehensive services. Collaborates with schools, community partners, service providers, and other stakeholders to coordinate services and support for children and families. Work closely with the Family Stability Coach.
- Case Management: Support households experiencing housing instability by empowering participants to achieve housing stability and self-sufficiency through a comprehensive multigenerational approach. The Housing Stability Coach will focus on areas such as housing, employment, education, health, and overall well-being. Maintains regular in-person contact to provide ongoing support and guidance.
- Data Management: Provides case management by updating family files according to agency standards. Logs all family contacts in CAC’s Client Management System. Analyzes, monitors, and tracks reports for progress on identified goals.
- Data Analysis: Participate in assessments as required by the grants.
- Other duties as assigned
What You Bring
- Bachelor's Degree with major course work in the social or behavioral sciences, social work, or an equivalent combination of education and experience.
- 1 years of case management experience
- Valid driver's license
- Able to work in an agile and evolving work environment
- Able to work independently and collaboratively with others
- Position requires flexibility in role as work hours and workdays may be unconventional in order to meet child and family needs.
- Strong computer proficiency, especially with Microsoft Office 365 (Word, Excel, and PowerPoint) and Microsoft Teams
- (Preferred) CAP60 experience
- (Preferred) Project management experience
