**Job Summary:**
Family Options, Inc. is seeking Client Support Coordinators to provide habilitative assistance and care to clients living in various Agency programs. The primary responsibilities include direct daily care, teaching, and supervising clients, as well as record-keeping.
**Responsibilities:**
- Directly supervise clients to ensure their well-being and location awareness.
- Assist in creating and implementing individual client plans and educational goals.
- Teach and assist clients with daily living activities such as hygiene, personal care, and community participation.
- Monitor client progress on goals and report any incidents of abuse or neglect.
- Complete incident reports, medication error reports, and implement behavioral plans.
- Collaborate with other staff, agencies, families, and consultants to provide comprehensive care.
- Maintain accurate records of services provided and client progress.
- Attend staff meetings, in-services, and required trainings.
- Administer medications, provide routine healthcare, and first aid.
- Keep program sites clean and organized, including transportation of clients when needed.
**Qualifications:**
- Successful completion of Medication Certification Program.
- Completion of PMT, CPR, and First Aid training.
- Possession of a valid CT driver's license with a satisfactory driving record.
**Requirements:**
- High School Diploma or equivalent required.
- Minimum of two years of experience working with individuals facing developmental and/or behavioral challenges preferred.
- Ability to lift up to 75 pounds and exert in excess of 100 lbs. of force if needed for physical restraint procedures.
- Proficiency in English reading, writing, and communication skills.
- Basic mathematical abilities including arithmetic with whole numbers, fractions, and decimals.
- Strong reasoning skills and problem-solving abilities.
