What are the responsibilities and job description for the Hospice Care Transition Manager position at Gentiva Health Services?
We are seeking a highly skilled Hospice Care Transition Manager to join our team at Gentiva Health Services. In this role, you will be responsible for ensuring the intake and referral processes are performed effectively and timely to meet the needs of our patients, families, and referral sources.
You will work closely with our business development team to complete the admissions process on referred patients in emergency cases, as staffing indicates, and for inpatient hospice admissions. Your key responsibilities will include providing accurate and timely information regarding hospice services to anyone inquiring, receiving calls from referral sources, responding timely, and initiating paperwork to begin Intake/referral process.
