Key Responsibilities
- Establish and nurture trusting relationships with caregivers while working closely with service providers to address client needs.
- Conduct comprehensive assessments of clients’ medical, social, educational, and psychological requirements to formulate personalized care plans.
- Develop and execute holistic care plans that cater to the distinct circumstances of each client.
- Supervise and coordinate the delivery of services, ensuring optimal resource allocation.
- Conduct in-person interactions and deliver a minimum of two client-centered health home services monthly.
- Accurately document all activities and adhere to quality assurance standards.
- Engage in ongoing training sessions, staff meetings, and in-service programs to maintain a high level of readiness and knowledge.
- Address quality assurance feedback promptly to improve service delivery.
- Proficiency in Microsoft Office Suite (Word, Excel, Outlook) and familiarity with electronic health record systems.
- Exceptional communication, organizational, and time management abilities.
- Capacity to manage a caseload of approximately 20 clients.
- Bilingual proficiency in Mandarin and English is essential.
- A Bachelor’s degree in Psychology, Sociology, Social Work, Human Services, or a related discipline.
- A minimum of two years of experience in case management or medical-focused care coordination.
- A valid New York State ID or driver’s license with the ability to travel locally.
- Commitment to working in-person at our Manhattan office during the training period, with weekly staff meetings.
