Job Description:
We are looking for a qualified, dedicated and experienced Hospice Admissions Coordinator
to join our rapidly growing home health agency. The ideal candidate will work proactively with the referral unit to ensure that all plans and arrangements are in place for the patients’ admission to hospice care.
Duties and Responsibilities:
- Responsible for performing all Intake duties for new patient’s referrals such as patient’s demographics, insurance verification, and ensure accurate medical records within EMR.
- Collect data from referral sources, family and facilities as needed to complete pre-admission information.
- Perform initial welcome phone calls to patients and families.
- Obtain all required information for referral and admission to HCC Hospice.
- Schedule hospice admissions visit efficiently.
- Facilitate DME and pharmacy orders for new admissions.
- Communicate and collaborate with Hospice Team on all admissions.
- Assist with preparing materials needed to complete an admission visit.
- Facilitate safe transition to hospice care by coordinating all appropriate services with referral sources and outside agencies, ensuring that all plans are in place prior to the time of patient discharge from hospital/skill nursing facility.
Requirement:
- The ideal candidate MUST have hospice experience.
- Familiarity with Epic and Care Port is preferred
- Knowledge of HIPAA and Medicare regulations
- Bachelor's degree preferred.
