Description
Human Support Services is seeking to add a full-time Case Manager, to our premier team of mental health professionals to provide mental health treatment in Monroe County, IL. This position works with our supported residential housing clients/population.
ILLUSTRATIVE EXAMPLES OF POSITION DUTIES:
- Case Management
- Advocacy, referral and linkage to Community Support Services
- Community Education
- Documentation of clinical services
- Meet agency standards for billable services.
- Utilize a variety of therapeutic interventions to address clinical needs.
- Complete required data entry and paperwork in accordance with program and agency policies and procedures, including but not limited to opening requirements, assessments, treatment/recovery plans, progress notes, treatment authorizations and authorization of medical necessity.

Our starting wage $20.13 an hour. In addition, we are currently offering a sign-on bonus of $1,000. Full-time bonus will consist of $500 after 30 days of employment and an additional $500 after 6 months of employment.

Requirements
* Minimum of a Bachelor's degree in a Human Services field (Psychology, Social Work, Counseling, etc.)
* One year relevant job experience in case management preferred.
* Must be willing to work in a multidisciplinary team environment.
* Must have a valid driver’s license and be insurable for auto coverage.
