Community Health Workers will provide hands-on assistance to address identified unmet health-related social needs. Individuals will be identified within target populations and assigned to a CHW who will provide hands-on assistance to identify and meet social needs. The CHW will screen individuals for both social and medical needs including but not limited to food insecurity, utility needs, housing instability/quality, interpersonal violence, and transportation needs. This position will work hand-in-hand with other care team members to implement the program and help individuals address their social needs with community service organizations and resources that exist in the local areaWe believe in the power of community support, and we're looking for compassionate individuals to join us as Community Health Workers.

• Providing Support: Listen to clients' concerns and offer guidance and assistance.
• Building Connections: Act as a friendly ear, providing essential support and care.
• Offering Resources: Help clients navigate challenges related to health, finances, and community resources.

Preferred Requirements:

• Strong interpersonal skills; ability to communicate effectively and build rapport.
• Familiarity with community resources (food banks, housing, medical support etc).
• The capacity to navigate complex situations, identify solutions, and connect individuals with appropriate community resources.
• Familiarity with available social services, healthcare providers, and other support systems within the community.

Skills:

• Excellent written and verbal communication skills.
• Basic computer skills.
