Description
*MUST BE ABLE TO WORK Thursday, Friday, Saturday, Sunday and Monday 8:00am - 5:00pm.

Receive and process information inquiries and process all incoming referrals.

Maintains a master copy of the Admission Consent Forms and prepares packets and
keeps the prepared packets in the Intake Office.

Receives signed Consents from the ACs and complete the Consent Audit on all signed
documents, records the statistics and turns them over to designated clinical team member
within 24 to 48 hours of admission.

Performs all functions of Admission Coordinator position, as assigned.

Can navigate through EMR, email, referral database and other electronic communication.

Comprehends the hospice local coverage determination criteria set by Medicare and can
help identify medically eligible patients for hospice.

Participates in community health fairs as assigned by Director of Admissions.

Attends daily stand up and weekly marketing meetings along with monthly all staffs.

Prepares weekly marketing supplies and/or bereavement baskets for the ACs to deliver to
their referral sources but may be called upon to assist.

Presents the Medicare Hospice Benefit to the community, patients and families. This
requires a thorough understanding of the Medicare Hospice Benefit, Medicare Long Term Benefit, Medicare HMO Benefit and all levels of care and requirements for GIP admissions.

Assists Director of Admissions and Admissions Department Manager on daily tasks that need to be accomplished.

Supports the ACs out in the field with presentations, co-marketing opportunities, and patient/family meetings to review/sign hospice consents.

Successfully develops relationships in the field with key referral sources to ensure admission goals are met.

2. Receive and process all incoming referrals:
-
Complete referral/intake process form.
-
Contact attending physician / referral source if necessary to obtain supporting
documentation for IHC or NCC admission.
-
Verify pay source and negotiate all third-party insurance benefits.
-
In situations where third-party coverage appears to be insufficient to pay for hospice
services, gain approval from the IDM or member of the executive team.
3. Set up and implement a routine follow-up system for all not taken under care (NTUC) referrals: -
All NTUCs will be documented, including the reason for not taking the patient, and correct NTUC.

All NTUCs will be signed by the IDM.
Maintains master copies of the admission consents forms and prepare IHC and NCC consent packets.
Update the Intake Department Manager and NCC staff on current status of all referrals.
Prepare statistical reports of all activity related to the admission process as directed.
As part of the intake process, reports to be processed or generated on weekly, monthly, quarterly, or annual basis to review referral sources and trends.
Oversee and coordinate Daily Stand Up Report and End of Day Report.
