Job Description
We are seeking a skilled Registered Nurse to join our team as a Hospice Case Manager. The successful candidate will possess excellent clinical skills, strong communication abilities, and a passion for delivering high-quality patient care.
The ideal candidate will have at least one year of relevant experience in hospice care or home health services. They should be able to work effectively with patients, families, and healthcare professionals to develop and implement comprehensive care plans.
The responsibilities of this role include conducting ongoing assessments of patients' physical and psycho-social status, initiating and developing care plans in collaboration with the Interdisciplinary Group (IDG), and providing education and interventions to support effective pain management.
Key Responsibilities:
- Complete ongoing assessments of patients' physical status and psycho-social status
- Initiate and develop a plan of care in collaboration with the IDG and attending physician
- Provide education and interventions to support effective pain management
- Ensure equipment, supplies, drugs, and biologicals are available as needed
- Make necessary revisions to the IDG plan of care as patients' needs change
- Document patient services accurately and timely
Requirements:
- RN with active, unrestricted license in the State of Michigan
- At least one year of relevant experience as an RN
- One year of management experience preferred
- Current CPR Certification
- Reliable transportation with a valid state driver's license and current car insurance
