Description
Job Purpose:
- To provide comprehensive case management services to clients of the Therapeutic Foster Care (TFC) program at Lutheran Social Services of the National Capital Area. This includes assessing client needs, developing and implementing treatment plans, coordinating services with other providers, and advocating for clients' well-being. The ultimate goal is to ensure the safety, permanency, and well-being of children and youth in foster care while supporting foster families.
Key Responsibilities:
- Conduct thorough assessments of clients' needs, including their physical, emotional, educational, and social well-being. This involves gathering information from various sources such as the client, foster family, biological family, therapists, and school personnel.
- Develop individualized treatment plans for each client based on their assessed needs. These plans should outline specific goals, objectives, and interventions aimed at achieving positive outcomes for the client.
- Implement the treatment plan by coordinating services with various providers, such as therapists, psychiatrists, educators, and other community resources. This includes scheduling appointments, facilitating communication between providers, and monitoring the effectiveness of services.
- Provide ongoing support and guidance to foster families. This includes training, education, crisis intervention, and advocacy. The Case Manager acts as a liaison between the foster family, the agency, and other involved parties.
- Maintain accurate and up-to-date case records, documenting all client interactions, assessments, treatment plans, progress notes, and other relevant information. This ensures continuity of care and compliance with agency policies and regulatory requirements.
- Advocate for clients' rights and needs, ensuring they have access to appropriate services and resources. This may involve representing clients at meetings, communicating with schools and other agencies, and navigating complex systems.
- Participate in regular supervision and team meetings to discuss cases, share best practices, and receive support. This fosters collaboration and professional development.
- Facilitate family engagement and work towards reunification or other permanency goals for the child, whenever possible and appropriate. This includes supporting visits between the child and their biological family, and working with the family to address any barriers to reunification.
- Ensure compliance with all relevant agency policies, procedures, and licensing regulations. This includes maintaining confidentiality, adhering to ethical guidelines, and reporting any suspected abuse or neglect.
- Participate in on-call rotation to provide 24/7 support to foster families and clients in crisis situations. This requires flexibility and availability to respond to emergencies as needed.
Qualifications
Required Education:
- Bachelor's degree in Social Work, Psychology, or a related field.
Required Experience:
Spanish/English requirement-
Minimum of two years of experience in case management, preferably in a non-profit setting.
- Experience working with diverse populations, including individuals and families facing various challenges.
- Demonstrated experience in conducting assessments, developing care plans, and providing supportive counseling.
- Experience collaborating with community resources and agencies to coordinate services for clients.
- Proven ability to maintain accurate records and documentation.
