JOB SUMMARY: To promote the mission of PATH to help individuals achieve a more independent and fulfilling life by managing an effective team to provide quality services. Responsible for the supervision of assigned staff and training and support of individuals in the program.
PHYSICAL DEMANDS: Lifting and transferring individuals, as needed.
QUALIFICATIONS: BA Degree in Human Services or AS Degree or 60 credits from an accredited college; At least one year experience, including supervisory, working directly with people with intellectual disabilities; Driver's License and personal vehicle; Excellent clinical, interpersonal and writing skills. Thorough knowledge of Intellectual Disabilities Field and Best Practices
SPECIFIC DUTIES:
Provide effective supervision of assigned staff and individuals adhering to the individual ratiosTrain staff in techniques and approaches that positively support individuals
Complete performance appraisals of assigned Adult Day and Older Adult Services staff
Assume primary programmatic responsibility in the absence of Program Manager or Program Director
Meet regularly for supervision with your supervisor
Participate in the interview and hiring process for Day Specialists as vacancies occur
Conduct orientation of new staff to assure that staff are knowledgeable about individual's needs and servicesPlan for new and improved services in accordance with best practices
Meet productivity standards of Adult Day and Older Adult Services
Maintain and enforce compliance with DPW/ODP and Older Adult Services Licensing and ensure minimal citations
Consistent with licensing regulations and the duties of a Program Specialist, complete and document the following duties:
Serve as Program Specialist for assigned cases and maintain their individual records in compliance with agency, DPW and Older Adult Licensing standards
Coordinate, complete and review assessments for assigned individualsDistribute the assessment and DP1050 (as appropriate) and transmittal 30 days before the ISP meeting
Attend the ISP and ISP update meetings and participate in the development of the ISP, ISP Annual Update and all ISP revisions
Fulfill the role of the Plan Lead, as needed
Review the ISP, ISP Annual Update and all ISP revisions for accuracy and report content discrepancies to the support coordinator and plan team members
Supervise, monitor and evaluate that services are provided as written in the ISP
Coordinate the services provided to the individuals including training and activity planning in accordance with the ISP
Review, sign and date the monthly documentation of an individual's participation and progress toward outcomes
Report changes related to the individual's needs to the supports coordinator as applicable, and plan team members
Review the ISP with the individuals as required and document that review
Provide documentation of the plan review to the supports coordinator as applicable and the plan team members
Inform the individual of the option to decline the ISP review
Recommend revisions to services our outcomes in the ISP to the Supports Coordinator and plan team members
Coordinate the training of Day Specialists to the content of health and safety needs relevant to each individual
Provide fire safety training to DPW individuals on an annual basis and for Older Adult Services individuals on a quarterly basis
Develop and implement quarterly reviews and monthly summaries for assigned individuals Attend, as needed, individual team meetings, QEST reviews, SIS meetings, Visions for Equality monitoring and Supports Coordination monitorings
Ensure the cleanliness and orderliness of site is maintained at all times
Appropriately schedule and ensure proper usage of the agency/program vehicles
Monitor leave time of assigned staff and ensure program coverage In conjunction with Human Resources, participate in related investigations
Refer all requests for references to Human Resources due to liability reasons
Carefully review, follow up, correct and process all incident reports generated by Adult Day and Older Adult Services staff in a timely fashion
