Weekend Unsheltered Community Navigator / Case Manager

Overview
Aspen Recovery Resources is currently hiring a full time weekend Unsheltered Community Navigator / Case Manager to join our team. In this role, you will work closely with individuals, struggling with homelessness providing necessary support, connecting clients to appropriate resources and filling out daily documentation. Ideal candidate is a compassionate and dedicated individual with experience working in behavioral health who is committed to enhancing the quality of life for those we serve.

The Community Navigator will work collaboratively with various stakeholders to ensure that clients receive the support they need to thrive.

Duties

• Assess client needs and develop personalized action plans to address barriers to services.
• Provide guidance and support in accessing resources such as housing, healthcare, workforce development, and addiction counseling.
• Facilitate connections between clients and community organizations, ensuring a smooth transition into available programs.
• Employ motivational interviewing techniques to empower clients in their journey toward self-sufficiency.
• Assist clients with various basic needs, including housing applications and behavioral health services.
• Conduct outreach activities to raise awareness of available resources within the community.
• Collaborate with social workers, crisis management teams, and other professionals to coordinate comprehensive care for clients.
• Maintain accurate records of client interactions and progress towards goals.
• Attend weekly Case Management meetings.

Requirements

• Bachelor’s degree in social work, psychology, or a related field is preferred; relevant experience may be considered in lieu of a degree.
• Strong understanding of addiction counseling practices and workforce development initiatives is preferred.
• Experience in behavioral health settings is highly desirable.
• Proven skills in crisis management and conflict resolution are preferred.
• Excellent communication and interpersonal skills, with the ability to build rapport with diverse populations.
• Familiarity with community resources or ability to learn.
• Ability to work independently as well as collaboratively within a team environment.
• Commitment to maintaining confidentiality and adhering to ethical standards in client interactions.

Requirements:

• Must have valid Driver’s License with an acceptable driving record.
• Must be living locally within a reasonable distance from Aspen CO or be willing to relocate.
• Must be flexible with shift work coverage. No set schedule can be guaranteed at first.
• Must be able to work scheduled shifts, to include weekends and holidays as needed, or on a rotating schedule.
• Must pass a pre-employment drug screen, reference check and CBI background check.
• Must adhere to the company's non-discrimination policy.

We currently need Friday-Saturday and Sunday coverage which can be accomplished with 12 hour shifts. However 4 ten hour shifts is also considered. Overtime opportunity is also available.
