We are currently seeking a full-time Pediatric Case Coordinator. Responsibilities include: providing care coordination for children with multiple or severe chronic medical conditions including but not limited to the following: cerebral palsy, seizure disorders, severe asthma, developmental delays or physical disabilities, chronic cardiac conditions, newborns born to high obstetric risk patients, and pediatric patients with other chronic diseases, Comply with state of Wisconsin requirements for child care coordination, work as a member of the Women’s Wellness Team which will involve cross training between the different roles, etc.
Successful candidates will have as a minimum: a Medical Assistant degree or experience in health education or other health-related field, experience working with pediatric populations preferable, the ability to present information to groups, excellent oral and written communication skills, and will be self-motivated, hard-working, and independent. Must be bilingual in Spanish/English, valid Wisconsin driver’s license, use of an automobile, and current automobile insurance.
