Qualifications
The Training Coordinator position is a multipurpose position requiring at least a Bachelor’s Degree in a health/human services, education, communications, or related field. Minimum of 1-year experience working in human and social service systems supporting children and families with long-term, chronic health conditions or developmental disabilities required. Prior Health Home Care Management experience is desirable, along with experience in training/curriculum development and implementation, and the use of electronic health record systems. Excellent organization, time management, and communication skills are essential to the position.
Hours: Full-Time 37.5 Hours/Week
Position Overview
The Training Coordinator functions as an important and versatile member of the Health Home Care Management team, primarily responsible for the ongoing new hire, agency-wide, and Health Home specific training/tracking, along with additional responsibilities in support of care management staff and administrative tasks as assigned by the Supervisor. The Training Coordinator provides support and guidance to staff to ensure the best possible personal and valued based outcomes are achieved for our clients, their families, and the organizations with which we interact. The Training Coordinator is responsible for conducting and tracking agency-wide training including but not limited to: New Hire Orientation, HIPAA/Compliance/Privacy & Security, Incident Reporting, along with tracking of yearly Mandated Reporter, Sexual Harassment, Harassment-Free Workplace, Safety in the Community, and CANS-NY training/certifications for staff. The Training Coordinator is responsible for managing the agency-wide training tracker/spreadsheet, individual staff training checklists, and tracking of training in SKIP Apps Database. The Training Coordinator must work with all care management program and supervisory staff to ensure the following training are offered and completed promptly: Health Home Care Management & CANS-UAS-MAPP system overviews, CANS-NY online and in-person training, Consents, Health Home Policies and Procedures, Care Management 101, Cultural Competency/Awareness, LGBTQ considerations supporting youth and families, person-centered planning, meeting facilitation, trauma-informed care, outreach/motivational interviewing, plan of care, comprehensive assessments, table of contents, case note documentation and billing processes, Medical and Therapeutic Service Orders, Level of Care, Children’s Waiver, quality assurance, nursing/home care, Medicaid managed care, premium reimbursements, various Early Intervention training, along with other training that becomes mandated by SKIP, NYS or our Health Home partners.
The Training Coordinator must also act as a Netsmart/GSI Champion trainer for SKIP to train staff on the appropriate use of those electronic health record platforms.
Additional Responsibilities
o Provide overall ongoing development/improvement/implementation of agency-wide training curriculum for all SKIP offices
o Provide new employees with initial and ongoing training
o Work with staff to set up and maintain their access to the Health Commerce System
o Notify staff of upcoming training. Sign staff up for any required external/internal training and monitor to ensure completion. Involve Supervisors/Directors as needed if staff is not completing training as required.
o Obtain, maintain and track relevant training certificates and training sign-in sheets for all Health Home program staff
o Maintain agency-wide training tracker/spreadsheet, track training in SKIP Apps Training Database, and ensure all individual staff comply with mandatory training requirements
o Conduct group and 1:1 pieces of training, and support the professional development of staff
o Educate staff on new services/resources that may be of benefit to families
o Participate in department staff meetings. Help conduct agency-wide all-staff training as needed, along with assisting to schedule relevant presentations
o Maintain training presentations, videos, and related resources on the SKIP network in an orderly and accessible way, helping make updates to training as changes occur
o Model effective communication and leadership skills
o Support care management staff to maintain neat and presentable physical & electronic case records for each person served, maintaining quality/audit-ready records
o Assist with internal audits on an as-needed basis
o Understand and follow all SKIP and program-related policies and procedures, demonstrating a command of regulatory requirements
