Position Overview: We are seeking a dedicated Case Manager to join our team, playing a crucial role in providing Case Management services. Your responsibilities will encompass personnel management, service provision, billing, training, and various miscellaneous tasks, all aimed at ensuring the well-being and support of individuals with developmental disabilities or those with long-term care needs.
Responsibilities
Personnel:
- Submit timecard and reimbursement sheet according to policy.
- Attend scheduled in-services and unit meetings.
- Maintain insurance binder and social work license records up to date.
- Conduct interviews, gather assessment data, and compile necessary information for case files.
- Establish contact with individuals served within required timelines.
- Obtain consent for the Release of Confidential Information.
- Provide Privacy Practices and relevant policies to individuals served.
- Facilitate Support Team meetings and assist in defining intervention plans.
- Advocate for individuals served in exercising citizen rights.
- Monitor and evaluate the effectiveness of services provided.
- Track time spent for individuals served and complete monthly billing reports.
- Address billing issues and assist in requesting additional case management units.
- Meet training requirements and maintain professional licensure status.
- Seek consultation for complex cases and stay informed of applicable rules and regulations.

Attend county-facilitated meetings and meet regularly with the Program Manager.

Ensure compliance with Lead Agency, DHS, and Thomas Allen standards.

Perform other assignments under the direction of Program leadership.
Minimum Qualifications:
- At least 18 years old.
- Demonstrated competency in job responsibilities.
- Background clearance for licensing agencies.
- Valid Driver's License and clean driving record.
- Bachelor's degree in social work or related field with licensure (or equivalent).
