MDS Coordinator

We are seeking a full-time MDS Coordinator (Minimum Data Set Nurse) to join our Nursing Administration Team in Manchester, NH. This role involves managing the RAI/MDS process, ensuring compliance with Medicare and Medicaid regulations, and providing education on person-centered care planning. Candidates must be a Registered Nurse (RN) with a valid New Hampshire license, have at least 2 years of long-term care experience, and possess strong clinical and communication skills.
