Contribute as a team player in a multidisciplinary team to plan and deliver employment support services for clients with acquired physical disabilities.
Provide personalised job coaching, including:
Pre-employment preparation
On-the-job training
Regular goal-setting and progress reviews
Building and facilitating support networks for clients
Conduct vocational assessments and job exploration activities.
Support clients in job search and placement by:
Advocating for their strengths and needs
Engaging and educating potential employers on internship and employment issues for clients.
Tap into relevant schemes and subsidies available to support persons with disabilities in their return-to-work journey.
Coordinate all aspects of the job placement process, including:
Workplace assessments
Employment negotiations
Job task analyses
Communication with stakeholders, including allied health and medical professionals
Plan and facilitate groups to support clients’ employment goal
Take on Case Manager responsibilities on a rotational basis, leading the care planning and progress of clients in the programme.
Maintain accurate and up-to-date records on client assessments, interventions, job placements, and ongoing support.
To equip onself with knowledge about local laws, policies, and guidelines related to return-to-work and employability.
Organise and participate in outreach activities to engage stakeholders in the training and employment of persons with disabilities.
Monitor programme outcomes by tracking and analysing data aligned to performance indicators, and provide progress reports to management.
Work closely with SPD’s Employment Support Programme (ESP) to maximise resources and client outcomes.
Carry out any other duties as assigned by the Programme Head.
