We are currently seeking a Peer Recovery Specialist that will work effectively within the WindRose Health Network culture, which is described as family-friendly, supportive, transparent, and treats employees as we would want employees to treat our patients. Position Summary:
- Assists patients in the community, or clinic setting that are identified as having a substance use disorder or are at risk of for a substance use disorder. Communicates to patients the purposes of specific behavioral/medical health recommendations and the impact it may have on their wellbeing. Helps patients identify how substance use affects their overall health and develop health/social management plans and goals.
- Complete screening, brief intervention, and referral to treatment for patients at Windrose identified as high risk for an alcohol of substance use disorder.
- Outreach during early recovery to patients at Windrose Health Network that are identified as having a substance use disorder or are at risk of a substance use disorder.
- Assist patients in early recovery with creating personal recovery plans.
- Assist patients at Windrose Health Network in accessing community resources.
- Facilitate substance use education and support groups both in the clinic and in the community.
- Provide education and guidance to patients in recovery.
- Promote accountability and provide support to patients in recovery before, during, and after treatment.
- Help build positive social skills in patients struggling with substance use disorders by role modeling.
- Provides support and advocacy during initial behavioral health visits or when necessary to assure patients' medical needs and referrals required are being conveyed. Follows up with both patients and providers regarding health/social services plans.
- Continuously expands knowledge and understanding of community resources and services. Facilitates patient access to community resources, including locating housing, food, clothing, prenatal classes, parenting, and providers to teach life skills, and relevant behavioral health services. Assist patients in utilizing community services, including scheduling appointments with social services agencies and assisting with completion of applications for programs for which they may be eligible.
- Facilitates communication and coordinate services between providers and the patients. Coordinates and monitors services, including comprehensive tracking of patients' compliance in relation to care plan objectives.
- Works collaboratively and effectively within a team. Establishes positive, supportive relationships with participants and provides feedback to other members of the team. Builds and maintains positive working relationships with the patients, providers, care managers, supervisors and office staff, from diverse cultural and socio-economic backgrounds. Works to reduce cultural and socio-economic barriers between patients and institutions.
- Travels extensively to community locations, various agencies, and other outreach destinations.
- Performs miscellaneous job-related duties as assigned.
- Bilingual in Spanish is preferred.
- Must have Certified Addiction Peer Recovery Coach certificate
